- Who can join: Participants must be 18+ years old. You may answer for yourself or for your child (aged 13+).
- Opt-In: Provide your name, postal code, and email address.
- Get Your Score: Receive your oral health score and browse the list of partner dental clinics.
- Book Your Appointment: Contact your preferred clinic to schedule your check-up, and don’t forget to show your oral health score upon arrival.

- Eligible patients include the participant (18+), their child (13+), or any immediate family member.
- Note: All appointments are subject to the dentist’s availability.
- The free offer is limited to a basic dental check-up only.
- Oral prophylaxis (cleaning) or other services are not included. Any additional procedures shall be separately arranged between the patient and the dentist.
