How to Apply for and Renew Your Philippine Passport: A Step-by-Step Guide

How to book an appointment online

Book an appointment online through passport.gov.ph/appointment. NOTE: Due to COVID-19 restrictions, no walk-in applicants will be accepted. Senior citizens, PWDs, pregnant women, minors below 7-years-old, and OFWs must now book appointments beforehand.

Choose a DFA site you want to visit, pick a date and any of the listed timeslots. Be warned that once you’ve chosen a slot, you must accomplish the following steps quickly.

Correctly fill out the forms you’ll be given regarding personal information, family information, application details, and contact number. When supplying your email address, it’s best that you use GMail or Yahoo! instead of custom work addresses. Make sure that it’s valid, active, and accessible as appointment confirmation will be sent there.

You will then be asked to pay.

  1. Choose between Regular Processing (10-15 days in Manila, PHP 950) or Express Processing (5-7 days in Manila, PHP 1,200). Both may take much longer if appointments are made in regional offices.
  2. Take note of the reference number that has been emailed to you.
  3. Pay via credit card or debit card, or proceed to the following payment centers for over-the-counter transactions:
    • Bayad Center
    • EcPay
    • Pera Hub
    • Robinsons Business Center and Department Stores
    • Waltermart Department Store
    • 7-Eleven
    • USCC (Western Union)
    • Villarica Pawnshop

Once payment has been confirmed, a PDF containing all the forms you need for your appointment will be sent to your email. It must contain the following: a checklist that indicates your appointment schedule, your application form, and two copies of your electronic receipt.

PRINT all the pages on A4 paper and show up to your schedule on time (or 30 minutes before) with the necessary documents and IDs.

Remember to bring face masks, face shields, and alcohol as well.

NOTE: In case of mistakes made when filling up the application form, you may have them corrected on the day of your passport application appointment. Inform your passport processor right away.

Passport Appointment Required Documents and IDs

For New Applicants (Adult):

  1. Original and photocopy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper
    • Married Females (who are using their spouse’s last name) must also present Original and submit photocopy of PSA Authenticated Marriage Contract on Security Paper or Report of Marriage
    • Local Civil Registrar Copy is required if PSA Birth Certificate is not clear or cannot be read
  2. Any of the acceptable IDs with one (1) photocopy

For New Applicants (Minor):

NOTE: Personal Appearance of minor applicant and either parent or authorized adult companion is required.

  1. Philippine Statistics Authority (PSA) Birth Certificate
    • Local Civil Registrar Copy may be required if PSA Birth Certificate is not clear or cannot be read
  2. Marriage Certificate is required if only one parent is accompanying the child
    • If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child
    • If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate
  3. Passport or Valid Government issued ID of either parent with one (1) photocopy
  4. School ID (if applicable)

For Renewal (Adult):

  1. Current ePassport with photocopy of data page
  2. Original PSA authenticated documents that will support the change of name, if applicable.
    • Marriage Contract
    • Annotated Birth Certificate
    • Annotated Marriage Contract to show annulment/divorce/court-ordered instruction
    • Death Certificate of Spouse

For Renewal (Minor):

NOTE: Personal Appearance of minor applicant and either parent or authorized adult companion is required.

  1. Current ePassport with photocopy of data page
  2. Marriage Certificate is required if only one parent is accompanying the child
    • If parents are unmarried, an Affidavit of Support and/or Consent executed by the Mother will be required if she is not accompanying the child
    • If Affidavit of Support and/or Consent is executed abroad, it must be authenticated by the Philippine Embassy or Consulate
  3. Original and photocopy of proof of filiation and/or guardianship
  4. Passport or Valid Government issued ID of either parent with one (1) photocopy
  5. School ID (if applicable)

List of Acceptable IDs

  • Social Security System (SSS) Card
  • Government Service Insurance System (GSIS) Card
  • Unified Multi-Purpose Identification (UMID) Card
  • Land Transportation Office (LTO) Driver’s License (Student Permit may be accepted if in card format)
  • Professional Regulatory Commission (PRC) ID
  • Overseas Workers Welfare Administration (OWWA) E-Card
  • Commission on Elections (COMELEC) Voter’s ID or Voter’s Certificate issued from COMELEC main office in Intramuros, Manila (Certificates issued in COMELEC’s regional offices may require further verification. Applicants are requested to present NBI or police clearance along with the Voter’s Certificate.)
  • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
  • Senior Citizen ID
  • Airman License (issued August 2016 onwards)
  • Philippine Postal ID (issued November 2016 onwards)
  • Seafarer’s Record Book (SRB)(must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)
  • Valid or Latest Passport (For Renewal of Passport)
  • For minor applicants, School ID OR Certificate of Enrolment with a photo of the minor and dry seal of school may be presented

Core Requirements for Passport Releasing

You will be asked to choose between claiming your passport onsite or have it delivered by a courier for an additional fee of PHP150 per applicant.

The following must be presented when claiming your passport:

  • Original DFA-issued receipt
  • Original acceptable ID

In case of a lost receipt, the applicant or authorized representative is required to present the Original notarized Affidavit of Lost Receipt executed by the applicant. Passports unclaimed after six (6) months will be canceled per Department Order No. 37-03.

NOTE:

  • In case the applicant is unable to claim his/her passport, immediate family members* are allowed to claim the passport on behalf of the applicant
    • Authorization letter (must mention affiliation with the applicant),
    • Original DFA-issued receipt,
    • Valid ID (original and photocopy) of applicant and representative
      *Immediate family members of legal age include parents, brother, sister, spouse, or children
  • In case a person other than an immediate family member will claim the applicant’s passport
    • Original notarized Special Power of Attorney (SPA) executed by the applicant, issued to the authorized representative
    • Valid ID (original and photocopy) of applicant and representative indicated in the Special Power of Attorney
  • For minor applicants
    • If passport will be claimed by the mother, the claimant must present an original DFA-issued receipt and valid ID
    • If passport will be claimed by the father, the claimant must present an original DFA-issued receipt, valid ID, and PSA documents indicating proof of parental authority over the minor
